That indicates its not tune related, more like o2 sensors not plugged in or wired wrong. Nothing wrong with o/s.

MAF / SD-only has nothing to do with open or closed loop.

Faulted O2s can put you in open loop. I have seen the O2s
get "pinned" to 450mV from some faults but I have some
folks on LS1Tech disputing this. Still, I know what I've seen.
But I can't say I have a good idea as to why, specifically
(what test or what kind of fault).

Might start with looking at how DTCs / codes may have been
"handled". Look at O2 sensor activity (waveforms) right after
clearing codes or writing a tune file, do they start off active
and then flatline to the middle? A dead sensor should flatline
near zero, stuck at midrange is the PCM's doing.

Customer may have checked "everything" that occurred to
them. Customer is probably not an expert and clue-quantity
    is highly variable.

People always insist nothing wrong with it till you show them otherwise. O2 sensors read 450mv when unplugged. If they were plugged in and working but just not going into closed loop you would see them change, this is where the log would be useful. I've looked through your tune, it's not a dtc or settings issue.

02 sensors are not wired correctly or not plugged in. Whats up with the speed sensor?
