Stays in Cold Start Mode and idle issues

On scanner the PID for cold start stays on Yes even after engine is up to normal temp. Also my MAF reading is almost at 10 g/s at idle but won't read on the graph. also my IAC stays at 310 all the time. What causes the computer to stay in Cold start mode? I am following the idle tuning guide by Rinkrat456. Also after i put it in closed loop mode on cold start it back fires when try to start but in open loop it does not.

2003 2500hd 6.0 / NV4500
engine stock except has a BTR truck torque cam, cold air intake and shorty headers.

There is no IAC on ETC vehicles so PID will report 310. No reason to log something that isn't there.
