One issue may be the LS3 TB compatibility with the truck TAC module. Are you using an XLink? Also, I think the values for the injector size are incorrect. I haven't been able to look at your log, but what are the fuel trims at idle?

Just checked and it is 29.5 in the stock file, but you also need to put all the injector offsets and low pulse adders from the TBSS file as well. That will make the fuel calculations more accurate.

In the tune repository on the website. Use the 06 TB files as the table formats are nearly the same.

http://www.hptuners.com/ Put in your serial # and password than you can go to the tune repository and look up the trailblazer tune.
