Why will my car not run right?

I have got the car tune well over 15 time within the last 2 months. When the car is at idle it runs kinda bad but then when you drive it will act like its running on half the cylinders then it will try to die out. Can anyone tell me whats going on?. If you need to know anything else just tell me what you need.

Your LTFT's are very high. +25 which is what your log is showing, indicates you may have an air leak somewhere. Check the hoses/vacuum lines and manifolds.
