The 680 torque you see is a command, not a calculation. The calculated torque is much closer to your dyno sheet @ 480ish max.
If it has the correct cam gear in it, and a GM sensor, and nothing is wrong mechanically it has to be in the wiring. First culprit is going to be the connector and pins.
Setting all the fail parameters works in almost all GEN4 cases. They're not near as finicky as GEN3s. Some of those can be a real pain. No rhyme or reason, just one will be a PITA and the next one...

Need vehicle details. Is this a stock GM vehicle? Swap of some sort?
Can you be more specific on this? How close to open air are the O2 sensors?
Not a fan of 2 sensors in the same pipe. It'll never figure out the bank to bank fueling.
Save yourself more headache and buy a decent harness from a company that can answer your questions.

And BTW your AFR SHOULD swing from rich to lean back and forth in closed loop.
When it happens the throttle blade opens some, the MAP goes high, loses vacuum, and then it recovers and the blade closes up again. I'd be taking fuel out of cranking fuel and the open loop tables...
