Look at your Startup Cranking spark table. I had the same issue when I cammed my car and had to reduce the timing at the higher coolant temp ranges to -10 to -15* advance. Starts warm now no issue.

What are your injectors doing? Have you logged injector pulse width or duty cycle and overlaid it with the WBO2 readings? Also fuel pressure.
Yes the 2-3 can happen too quickly. Adjust the shift MPH up for that shift and down for the 5-6 until it shifts the way you want.
