The godzilla control pack manual strategy may work.
So use the 4.6l control pack tune.
I think you can flash the control pack strategy on a gen 1 and remove pats that way. But I haven't done it myself so verify.
Did you load a control pack tune or something else? Post the tune you loaded.
Nothing in the tune is really popping out, it is kind of a generic off the shelf tune.
Tuning out the LTFT should be easy enough, does the volant have a larger than stock MAF housing? If so do...
Posting a tune would help. Also verify the data in the tune matches the injectors in the truck.
I think there might be something else going on, watch your fuel pressure and see how long it takes to drop back down after priming or running. These should start fine with the standard prime timing.
Check that the axis values are the same.

I can't say why your having problems, my MVPI2 is plug and play with my 06. I would have expected yours to be as well. I think the reason no one has responded is the problem isn't all that common.

Check that your O2 wiring isn't swapped side to side. It is possible with the control pack wiring and will account for the trims.
I know you stated you have a crate engine, is it for sure a crate...

Yes, they will run a little rough for a few seconds on startup with E85 as well. It isn't a big deal though.
